    Multiple fractures are frequently encountered in adults following road traffic accidents. A 32-year-old male presented with multiple fractures in his right lower extremity, including a femoral shaft fracture, distal third fractures of the tibia and fibula, as well as a calcaneal fracture. The patient provided a history indicative of a road traffic accident. X-rays were performed on both hip joints, both knee joints, and the ankle joints. Treatment involved open reduction and internal fixation (ORIF) with interlocking nailing for the femur, tibia, and fibula, alongside ORIF with plating using a screw-out set (SOS) and cannulated cancellous (CC) screw fixation for the calcaneal fracture. Additionally, the Ilizarov procedure was conducted following debridement over the right foot. Post-surgery, the patient experienced primary symptoms of hip joint pain and restricted hip joint movement. Physiotherapy was initiated to address these issues. Evaluation of outcome measures indicated a reduction in joint pain, significant enhancement in joint mobility, and an increase in muscle strength.

    UNASSIGNED: Bilateral combined fractures of the greater tuberosity with anterior internal shoulder dislocation are extremely rare, presenting challenges in treatment due to their uncommon occurrence and lack of standardized management protocols.
    METHODS: We report the case of a 22-year-old patient who experienced bilateral anterior shoulder dislocations with associated fractures of the greater tuberosities following a seizure-induced fall. Clinical Discussion: The unique aspect of our case is the unusual mechanism of injury. Unlike typical convulsive seizures, which often result in bilateral posterior dislocations, our patient suffered bilateral anterior dislocations and fractures of the greater tuberosities. Therapeutic management of greater tuberosity fractures lacks a standardized approach, but surgical intervention is warranted if displacement exceeds 0.5 cm. Treatment options include open reduction and internal fixation (ORIF), percutaneous screw fixation, arthroscopic procedures with suture anchors for double-row repair, or a combination of arthroscopic techniques and screw fixation. ORIF with plate osteosynthesis may be recommended for comminuted fractures with large fragments. To assess functional outcomes, commonly used scoring systems include the UCLA Shoulder Score, ASES Score, and Constant-Murley Score.
    CONCLUSIONS: Bilateral anterior shoulder dislocation associated with combined fractures of the greater tuberosity is an extremely rare occurrence. Given its anatomopathological significance and impact on functional recovery, careful and individualized management of greater tuberosity fractures is crucial, considering its role as a point of insertion for the rotator cuff tendons.

    BACKGROUND: The incidence of osteoporotic pelvic fractures is increasing. The broken anterior pelvic ring is preferentially fixed with long intramedullary screws, which require a good understanding of the patient-specific anatomy to prevent joint perforation. The aim of this study was to assess the variability of the superior pubic ramus and the supra acetabular corridors\' length and width using statistical shape modelling.
    METHODS: A male and female statistical shape model was made based on 59 forensic CT scans. For the superior pubic ramus and the supra acetabular corridor the longest and widest completely fitting cylinder was created for the first 5 principal components (PC) of both models, male and female pelvises separately.
    RESULTS: A total of 59 pelvises were included in this study of which 36 male and 23 female. The first 5 principal components explained 75% and 79% of the pelvic variation in males and females, respectively. Within 3 PCs of the female statistical shape model (SSM) a superior pubic ramus corridor of < 7.3 mm was found, 5.5 mm being the narrowest linear corridor measured. Both corridors in all PCs of the male SSM measured > 7.3 mm.
    CONCLUSIONS: Within females a 7.3 mm and 6.5 mm screw won\'t always fit in the superior pubic ramus corridor, especially if a flat sacrum, a small pelvis or a wide subpubic angle are present. The supra acetabular corridor did not seem to have sex-specific differences. In the supra-acetabular corridor there was always enough space to accommodate a 7.3 mm screw, both in males and females.

    BACKGROUND: Cervical spine computed tomography (CSCT) scans are frequently performed in older emergency department (ED) trauma patients based on the 65-year-old high-risk criterion of the Canadian Cervical Spine Rule (CCR). We sought to determine the positivity rate of CSCT scans in symptomatic and asymptomatic patients to assess the current applicability of age in the CCR.
    METHODS: We reviewed CSCT ED reports from two institutional hospitals from 2018 to 2023. The primary variable was age; however, we also recorded fracture types and sites and type of treatments. Patients were separated into symptomatic and asymptomatic cohorts. We used a Fisher\'s exact test to compare variables between the asymptomatic and symptomatic groups and chi-square tests for comparison between age groups.
    RESULTS: Of 9455 CSCTs performed in patients ≥ 65 years, 192 (2.0%) fractures were identified (113 females); 28 (0.30%) were in asymptomatic patients. The rates of fractures (1.6%) and asymptomatic fractures (0.18%) were lowest in the 65- to 70-year age group. There were no distinguishing features as to the level or part of the vertebra fractured or surgical treatment rate between asymptomatic and symptomatic patients.
    CONCLUSIONS: Cervical spine fractures in posttrauma patients ≥ 65 years are uncommon, with the lowest incidence in those 65 to 70 years old. Excluding asymptomatic individuals aged 65-70 from routine CSCT presents a minimal risk of missed fractures (0.18%). This prompts consideration for refining age-based screening and integrating shared decision making into the clinical protocol for this demographic, reflecting the low incidence of fractures and the changing health profile of the aging population.

    Fanconi syndrome (FS) can present with hypophosphatemia, renal glycosuria, hypouricemia and aminoaciduria. Phosphate depletion is the most critical clinical aspect of FS as it leads to osteomalacia. Some patients present with symptoms and signs related to hypophosphatemic osteomalacia (HO). Thus, these patients present with these symptoms and are misdiagnosed. From an investigation of the published literature, HO symptoms are found to be non-specific and were thus misdiagnosed in various centers. The present study describes the case of a a 46-year-old male with FS who suffered from joint pain and was first misdiagnosed. After he was referred to the authors\' hospital, his case was evaluated and following a consideration of the results of this evaluation, he was diagnosed with idiopathic FS with multiple osteoporotic fractures. Furthermore, the present study performs a brief literature review other cases of patients that were misdiagnosed and whose symptoms were later found to be due to HO are also discussed. It is hoped that the present study may increase the awareness of HO among physicians and may help to draw attention to such cases of patients presenting with non-specific symptoms.

    BACKGROUND: Femoral neck fracture (FNF) accounts for approximately 3.58% of all fractures in the entire body, exhibiting an increasing trend each year. According to a survey, in 1990, the total number of hip fractures in men and women worldwide was approximately 338,000 and 917,000, respectively. In China, FNFs account for 48.22% of hip fractures. Currently, many studies have been conducted on postdischarge mortality and mortality risk in patients with FNF. However, there have been no definitive studies on in-hospital mortality or its influencing factors in patients with severe FNF admitted to the intensive care unit.
    OBJECTIVE: In this paper, 3 machine learning methods were used to construct a nosocomial death prediction model for patients admitted to intensive care units to assist clinicians in early clinical decision-making.
    METHODS: A retrospective analysis was conducted using information of a patient with FNF from the Medical Information Mart for Intensive Care III. After balancing the data set using the Synthetic Minority Oversampling Technique algorithm, patients were randomly separated into a 70% training set and a 30% testing set for the development and validation, respectively, of the prediction model. Random forest, extreme gradient boosting, and backpropagation neural network prediction models were constructed with nosocomial death as the outcome. Model performance was assessed using the area under the receiver operating characteristic curve, accuracy, precision, sensitivity, and specificity. The predictive value of the models was verified in comparison to the traditional logistic model.
    RESULTS: A total of 366 patients with FNFs were selected, including 48 cases (13.1%) of in-hospital death. Data from 636 patients were obtained by balancing the data set with the in-hospital death group to survival group as 1:1. The 3 machine learning models exhibited high predictive accuracy, and the area under the receiver operating characteristic curve of the random forest, extreme gradient boosting, and backpropagation neural network were 0.98, 0.97, and 0.95, respectively, all with higher predictive performance than the traditional logistic regression model. Ranking the importance of the feature variables, the top 10 feature variables that were meaningful for predicting the risk of in-hospital death of patients were the Simplified Acute Physiology Score II, lactate, creatinine, gender, vitamin D, calcium, creatine kinase, creatine kinase isoenzyme, white blood cell, and age.
    CONCLUSIONS: Death risk assessment models constructed using machine learning have positive significance for predicting the in-hospital mortality of patients with severe disease and provide a valid basis for reducing in-hospital mortality and improving patient prognosis.

    OBJECTIVE: Juxta-physeal fractures at the base of the proximal phalanx (FBPP) of the small finger are one of the most common hand fractures in children. Although many of these fractures are treated nonsurgically, it is unclear which fractures benefit from surgery or the degree of acceptable angulation appropriate for nonsurgical management. Our study aimed to assess long-term, patient-reported outcomes regarding function, appearance, and pain after nonsurgical management of FBPP of the small finger in a pediatric population.
    METHODS: Our hospital Picture Archiving and Communication Systems database was queried to identify radiographs of the small finger of children between the ages of 8 and 16 years old taken from 2011 to 2021. Displacement on initial injury radiographs was calculated using the diaphyseal-metacarpal head angle. Patient-reported function, appearance, and pain were measured using standardized assessment tools sent to patients and parents electronically. Optional clinical photographs were uploaded by parents and assessed for residual clinical deformity.
    RESULTS: One hundred eighty-one eligible subjects were identified. Eighty (44%) agreed to participate, and 40 (22%) parent and 38 patient surveys were completed. The mean age at the time of injury was 11 years old (8-14 years), and the mean age at the time of survey completion was 17 years old (11-23 years). Patient T-scores were higher than the average reference T-score on the Patient-Reported Outcomes Measurement Information System Short Form v2.0-Upper Extremity. Overall, greater than 78% of patients and parents reported appearance as 8/10 or above on a VAS. Ninety-five percent (108/114) of patients reported no pain (0/10) for pain during activities and at rest.
    CONCLUSIONS: Children with up to 26° of initial coronal plane angulation reported better function than a reference population, good appearance, and no pain, at a mean of 6 years after injury. Our findings support nonsurgical management of most FBPP of the small finger in children.

    UNASSIGNED: Despite the fact that China amounts to one-fifth of the world\'s population, has a higher proportion of the elderly, and has a higher prevalence of osteoporosis and fracture, limited studies have investigated the association between dietary patterns and bone mineral density (BMD) as well as fracture risk among the elderly Chinese population. We aimed to investigate the association between different dietary patterns and BMD as well as the risk of fractures, and this association may vary between elderly women and men.
    UNASSIGNED: Building upon the China Osteoporosis Prevalence Study, we included 17,489 subjects aged ≥40 years old randomly sampled across 44 counties/districts of 11 provinces or municipalities in China who completed a food frequency questionnaire. BMD was measured by dual x-ray absorptiometry. Vertebral fracture was defined based on lateral spine radiographs using the semi-quantitative technique of Genant.
    UNASSIGNED: A diet rich in \"carnivorous\", \"vegetarian\", \"dairy, fruit, and egg\" was significantly associated with higher BMD at total hip (TH), femoral neck (FN), and lumbar spine 1-4 (L1-4). Yet, a diet rich in \"beverage and fried food\" was associated with a lower BMD at the FN and L1-4. High quartiles of the carnivorous diet were associated with 34%-39% reduced risk of clinical fracture in the past 5 years and vertebral fracture. Stronger associations were observed among women. Sensitivity analysis among postmenopausal women presented even stronger positive associations between carnivorous and vegetarian diets and high BMD, as well as between carnivorous diet and reduced risk of fractures.
    UNASSIGNED: Our study suggested that a diet rich in meat, vegetables, and dairy, fruit, and eggs might be associated with greater BMD and a lower fracture risk, while beverage and fried foods may be associated with a lower BMD at L1-4, especially among elderly women. These findings are relevant to provide recommendations on dietary nutrition regarding the elderly population at high risk of osteoporosis and fractures, especially postmenopausal women.
