gender reassignment procedures

Case Reports
UNASSIGNED: In today\'s world, the concept of gender has been scrutinized and an appreciation for those who experience dysphoria with their birth sex and gender classification is becoming more commonplace. Keeping in mind the patients gender orientation in addition to their birth sex is necessary when assessing health conditions more prevalent in one sex, such as breast cancer.
UNASSIGNED: In this report, we present a 51-year-old African American transgender female with history of chemotherapy and mantle-field radiation treatment for sub-clavicular and mediastinal Hodgkin\'s lymphoma 24 years prior to presentation of a new left neck mass. The enlarged lymph node was removed revealing metastatic salivary adenocarcinoma with features corresponding to metastatic breast carcinoma. Computed tomography (CT) of the chest, abdomen, and pelvis detected metastasis to the pelvis and a few lucent bone lesions in the lumbar spine. Of note, the patient underwent free silicone injections into both breasts to emphasize her desired gender three years after treatment for Hodgkin\'s lymphoma. Based on her history of metastatic disease and history of mantle radiation, it was determined that her previous cancer diagnoses were likely due to metastatic breast cancer that was obscured by silicone injections. Bilateral skin-sparing mastectomy was performed, patient recovered well, and continued with palliative care at follow up.
UNASSIGNED: Even though there is significant data regarding the incidence of breast cancer in the separate female and male populations, review of the literature shows minimal information regarding incidence in the transgender population. Our hope is that this report will contribute to the current base of knowledge present in the literature while also bringing attention to the need for further investigation of sex-specific diseases in transgender individuals.

UNASSIGNED: Although many studies have shown that transgender women (TW) tend to have more depressive symptoms compared with the general population, the exact prevalence of major depressive disorder and contributing risk factors have not yet been studied, especially in those who have undergone gender-affirming genital surgery (GAS).
UNASSIGNED: Every TW aged 20-60 years attending transgender clinics in the Thai Red Cross Society for postoperative checkup were invited to complete a self-report questionnaire, including (1) demographic data, (2) the Thai Patient Health Questionnaire 9 (PHQ-9), (3) satisfaction with psychosocial factors (relationship in family, friends, and society), sexual experiences and outcomes of gender-affirming treatments before and after GAS, and (4) contributing factors to depressive symptoms. Correlations between demographic data and depressive scores were tested. Paired t-test was used to compare pre- and post-GAS data.
UNASSIGNED: A total of 97 TW were analyzed, with 22.7% considered to have \"major depression\" (PHQ-9≥9) after GAS. Postoperative improvement in satisfaction with psychosocial factors and gender-affirming treatment outcomes were observed, especially in sexual experience. Depression severity was found to be negatively correlated with age and sexual satisfaction. Notably, presence of male characteristics before surgery, poor social support and understanding, and postoperative surgical complications were greatly considered as factors contributing to depressive symptoms.
UNASSIGNED: Compared with non-GAS TW, depression among those who underwent GAS in Thailand was lower but still significant and was linked to younger age and poor postoperative sexual experiences. These findings were supported by the number of qualitative assessments in the psychosexual domain and postoperative treatment outcomes. Clinical Trial Registration Number - TCTR20190904003.

It is becoming increasingly common for people to openly identify as transgender, yet there is little research in the field of genetic counseling regarding this community\'s unique medical needs. Transgender patients are likely to present with issues that cross genetics and gender-related care in a cancer genetic counseling session, and empiric data about these differences is needed to provide adequate care. In order to investigate what specific health topics and concerns are addressed in cancer genetic counseling sessions with transgender patients, 21 cancer genetic counselors who have seen transgender patients were interviewed. Through inductive analysis, six themes emerged: (1) documentation systems are not inclusive or clear; (2) genetic counselors feel unprepared for these sessions; (3) gender affirming hormones impact risk assessment; (4) genetic testing affects gender affirming surgical decisions; (5) transgender patients present at younger ages to clinic; and (6) pathogenic variants allow for insurance coverage for gender affirming surgeries. This study\'s findings point to opportunities for the field of genetic counseling to enhance services for transgender patients by reporting distinctive situations that may arise in clinic with these patients and providing training recommendations for genetic counselors.
