    We report a case of a 41-year-old female patient presenting with watery diarrhoea and myalgia in the winter-season. Before her symptoms started she had participated in a pig slaughtering with her family. Some of the family members also became ill. On her physical examination periorbital odema and myalgia were found. Eosinophilia, hypalbuminaemia, elevated lactate dehydrogenase and creatin kinase levels were detected on laboratory investigations. The clinical picture, the laboratory findings and background epidemiological data implied the diagnosis of trichinellosis and albendazol was started. Serum gained on the 22nd post-infectious day turned out to be equivocal for trichinellosis. For this reason and because of the refractory fever a muscle-biopsy was done. Granulomatous myositis described by histology and Trichinella seropositivity from the repeated serum sample on the 62nd post-infectious day finally confirmed the diagnosis. During the course of the disease, we experienced elevation of troponin I suggesting myocarditis, but it was accompanied neither with abnormal ECG signs nor characteristic symptoms. Almost a century ago, a case report was published in Hungarian with a similar introduction. Trichinellosis in that epidemic setting led to the death of five people. Orv Hetil. 2019; 160(24): 952-957.
    Absztrakt: Vizes hasmenés és izomfájdalmak miatt vettünk fel infektológiai osztályunkra egy 41 éves nőbeteget a téli évszakban, aki a panaszok kezdete előtt családi disznótoron vett részt, ahol mások is megbetegedtek. Fizikális vizsgálattal periorbitalis ödémát és myalgiát tapasztaltunk. Laboratóriumi vizsgálattal eosinophiliát, hypalbuminaemiát, kreatin-kináz- és laktát-dehidrogenáz-emelkedést észleltünk. A fizikális és laboratóriumi vizsgálati eredmények az epidemiológiai adatok ismeretében trichinellosisra utaltak, ezért albendazol adását kezdtük. A 22. napon post infectionem levett savó szerológiai vizsgálata trichinellosis irányába kétes pozitív eredményt adott, ezért és a kezelés ellenére fennálló tartós láz miatt izombiopsziát végeztünk. A szövettan által leírt granulomatosus myositis és a 62. napon post infectionem megismételt szerológiai vizsgálat végül igazolta a trichinellosist. A kórlefolyás során szívizom-érintettségre utaló troponin-I-kiáramlást tapasztaltunk, amit myocarditisre jellemző klinikai tünet, illetve EKG-eltérés nem kísért. Közel száz évvel ezelőtt hasonló kezdetű esetleírással találkozhatunk, a trichinellosis akkor öt halálos áldozatot követelt. Orv Hetil. 2019; 160(24): 952–957.

    The examination of wild boars gained in Poland shows for the first time occurrence of Trichinella nativa, freeze-resistant species of Trichinella in this host from the central Europe region. This finding is not only one of several cases of T. nativa invasion in wild boars all over the world but also one of the very few cases of T. nativa detected so far beyond the known boundary of occurrence of this species. The molecular characterization of discovered larvae based on analysis of partial genes: 5s rDNA-ISR and CO1 confirm the findings. Moreover, the analyzed DNA sequences of both genes present new haplotypes of T. nativa in comparison to that described previously.

    The presence of the parasite Trichinella spiralis in humans does not always manifest itself with obvious clinical symptoms; the clinical manifestations of trichinellosis are polymorphic and can cause diagnostic difficulties. Our aim was to identify risk factors that can be linked to the severity of the disease. We conducted a retrospective analysis of 143 cases of trichinellosis admitted to the Infectious Disease Hospital in Braşov, Romania during 2001-2008. We found that children 10 and younger were less prone to exhibit medium or severe symptoms. Patients with leukocytosis had a 1.75 times higher risk of developing medium to severe symptoms relative to those with normal white blood cell counts. Patients with high eosinophil counts had a 2.05 times higher risk of exhibiting average or serious symptoms relative to those with low or normal eosinophil counts. Repeated consumption of contaminated meat increased the chances of developing discernible forms of the disease by 5.25 fold when compared to those who only occasionally ate meat contaminated with Trichinella. Regular consumption of raw or undercooked meat increased the chances of developing medium or severe forms of this disease by 1.67 times when compared to those who consume meat that had been thoroughly cooked.

    Diagnosis of occupational infectious diseases gives rise to a number of problems in the practice of occupational medicine. Similar symptoms of many infections, including parasitic diseases, can limit the range of applied diagnostic and certification procedures only to diseases induced by pathogenic agents or exposures occurring in the occupational environment. In some cases this makes difficult or even impossible to explain a real cause of patients\' complaints. We present a case report on the patient with suspected occupational borreliosis. Due to many diagnostic doubts during hospitalization, including long term presence of some clinical manifestations reported by the patient, which could occur in the course of borreliosis and also suggest earlier infection by trichinae, a wider span of specialist examinations was applied. Finally, the invasion of trichinae was diagnosed and later confirmed by serologic tests.

    Trichinellosis is one of the most serious foodborne parasitic zoonoses in Europe. Wild carnivorous and omnivorous hosts are the main reservoirs of Trichinella spp. nematodes in nature. In the winter of 2008-2009, an atypical clinical case of trichinellosis occurred for the consumption of pork from a wild boar (Sus scrofa) hunted in southwestern Alps in Italy. The symptomatic individual showed delayed development of oedemas in the lower limbs and eosinophilia, which appeared three months after infection. Muscle samples harboured 3.8 larvae/g, which were identified as Trichinella britovi. During the epidemiological investigation, anti-Trichinella IgG were detected in five hunters.

    Trichinella pseudospiralis is a non-encapsulated species infecting both mammals and birds. In Italy, this parasite was reported only in two night-birds of prey of Central Italy. In January 2010, Trichinella larvae were detected in three wild boars (Sus scrofa) of two regions of Northern Italy by enzymatic digestion. The parasites were identified as T. pseudospiralis by multiplex-PCR. The first infected wild boar was hunted in the Emilia Romagna region and the other two infected wild boars were bred outdoors in a small family farm of the Friuli Venezia Giulia region. These new epidemiological data reinforce the role of the wild boar as the main reservoir of T. pseudospiralis in Europe.

    Human neurotrichinellosis is seldom reported. This is likely the result of the low incidence of parasites from the genus Trichinella in the United States domestic food supply, as well as difficulties in diagnosing the disease, especially when neither the organism nor the source of the infection are readily available. Although trichinellosis from domestic food supplies has been decreasing for many years, a resurgence has occurred in cases derived from the consumption of wild game. We report a rare case of neurotrichinellosis in the United States and implicate wild game as the source of the infection. These results suggest that clinicians should consider the potential for Trichinella infection in cases where wild game is common in the diets of the patients.

    We report a single case of trichinellosis contracted in Algeria (Batna region), in a practising Moslim. Shortly after returning to France in November 2004, the patient developed the typical clinical and biological signs of the disease. Although the patient claimed having only eaten mutton, an unusual host for Trichinella, a meticulous investigation revealed that he also had eaten a grilled leg of jackal (Canis aureus). One of the four Trichinella larvae detected in a muscular biopsy enabled us to identify the parasite as Trichinella britovi by a multiplex PCR analysis. This is the first identification of the etiological agent of sylvatic trichinellosis occurring in North Africa and the first case of symptomatic trichinellosis due to jackal meat consumption in Africa.
