    Graft failure is a common postoperative complication after anterior cruciate ligament (ACL) reconstruction. Recently, a theory has emerged that histological and microstructural factors of autografts may be related to graft failure. We simultaneously collected the semitendinosus tendon (ST), quadriceps tendon (QT), and patellar tendon (PT) from a 22-year-old patient to provide insights into the differences in the collagen-type composition of the three tendons in skeletally mature patients. These findings may serve as a basis for selecting autografts for ACL to reduce graft failure rates. The patient was a 22-year-old female who required the removal of artificial ligament, screws, and washers and medial patellofemoral ligament (MPFL) reconstruction with an ST autograft after two surgeries for recurrent dislocation of the left patella. The ST, QT, and PT obtained during necessary intraoperative procedures were used as samples. The tissues were processed and immunostained; this was followed by confocal microscopy. Evaluation was performed by calculating the percentage of areas positive for collagen types I and III.The percentage of type I collagen in the ST, QT, and PT groups was 88%, 85%, and 88%, respectively.The collagen-type composition was examined following simultaneous collection of the ST, QT, and PT. The results revealed no significant differences in the content of physically strong type I collagen, which supports previous findings showing that the clinical outcomes after ACL reconstruction do not vary with the autograft used.

    UNASSIGNED: The extensor mechanism and patellar tendon (PT) are considered essential components. Adult PT avulsion from the tibial tubercle is uncommon, with little information in the literature. Technical challenges arise during injury management. Knotless anchors have several applications in treating tendon injuries, such as the rotator cuff, distal biceps, and quadriceps tendons but were not used to repair distal PT avulsions.
    UNASSIGNED: A 50-year-old male patient, an active adult, presented to emergency department with significant right knee pain, giving away and limitation of range of motion (ROM) that had started after he sustained direct trauma with a ground-level fall on his knee that morning.
    UNASSIGNED: In this paper, we report a case and describe a technique to manage a rare presentation of pure distal PT rupture without an avulsion fracture using knotless anchors with FiberTape®, which showed excellent results. To the best of our knowledge, this technique has never been used before in such injury and anatomical location. At 2 years of follow-up, the patient is free of complaints with almost full ROM at the knee and back to his standard daily life activity.

    Tuberculosis (TB), an infection caused by Mycobacterium tuberculosis, accounts for significant morbidity and mortality worldwide. While primary TB predominantly involves the respiratory system, approximately 10-19% patients have musculoskeletal involvement. We present a case of a 54-year-old year gentleman with insidious onset of anterior knee pain. Imaging demonstrated a soft tissue lesion involving the patellar tendon and eroding the inferior pole of the patella. The imaging features, particularly ultrasound, resembled those that are typically seen with gout. Ultrasound guided biopsy revealed this to be TB of the patella. The patient was successfully treated with antitubercular therapy. Musculoskeletal TB, while usually not a primary form of TB, is an important consideration particularly in patients with risk factors for the disease. Its imaging features can mimic other forms of arthropathy such as gout. Moreover, while the knee is a relatively common site for TB infection, isolated involvement of the patella and patellar tendon is extremely rare. The case highlights the possibility of rare musculoskeletal manifestation of TB as a well as a potential imaging pitfall of TB infection mimicking gout, which is an important consideration in clinical practice.

    UNASSIGNED: Rupture of the extensor apparatus is a serious complication that can occur in a prosthetic knee. Bifocal extensor ruptures are rare and even more uncommon in adults.
    METHODS: We report the case of an obese, diabetic, and hypertensive patient who underwent total knee arthroplasty two weeks previously and was admitted following a fall from her height for a rupture of the patellar tendon with release of the sutures. An investigation revealed bifocal avulsion of the patellar tendon from its patellar and tibial insertion. The patient was treated with double lacing and anchoring at the patellar and tibial levels with wire cerclage.
    UNASSIGNED: Several factors contribute to tendon fragility: degenerative changes linked to age, general or local conditions, and surgical approaches for TKA. These abnormalities also affect the tendon\'s ability to heal and should suggest the inadequacy of simple repair without tendon augmentation using a plasty, an autograft, or an allograft.
    CONCLUSIONS: The prognosis for patients with extensor tendon ruptures in the TKA (total knee arthroplasty) remains unclear, and in our case, this was exacerbated by the delay between prosthesis insertion and the incident and by the open nature of the lesion.

    Acute distal patella tendon avulsion from the tibial tubercle (TT) is a relatively rare injury that is usually described in the adolescents or elderly population in their 7th or 8th decades. Bifocal avulsion fractures of the patella tendon from the TT and the distal pole of the patella are exceptionally rare in adults. In this case report, we present a 52-year-old healthy old male who was treated for bifocal avulsion of the patellar tendon with open reduction and internal fixation augmented with two ULTRATAPE sutures. To our knowledge, this is the first case report to describe this injury in a healthy middle-aged patient.

    Patella fractures correspond to 1 % of all fractures. In recent decades, the role of the patella in increasing the lever arm of the quadriceps has been well defined. Surgical treatment is indicated for open fractures, those that compromise the function of the extensor mechanism, those with a joint gap >5 mm and/or joint incongruity >3 mm, a group that corresponds to around 30 % of the total. Anatomical reduction and stabilization with various types of modified tension bands is the most frequently used procedure. Biomechanical studies have shown that stabilization through the use of a tension band replacing the Kirchner wires with cannulated screws presents adequate resistance to fracture displacement and provides greater stability than the classic configuration, maintaining the theoretical principle of converting the forces of anterior tension of the patella generated by the quadriceps in compression at the level of the articular surface. A case of a patient who required reduction and osteosynthesis of an exposed patella fracture, associated with an extensor mechanism lesion, is presented. Clinical and radiographic characteristics of the patient and the resolution of the case are described.

    UNASSIGNED: Patellar tendon rupture is a rare injury and is often missed in primary and secondary care centers because of non-visibility in X-rays. Neglected rupture is even rarer and gives rise to significant disability. These injuries are technically difficult to repair with invariably poor functional outcomes. This needs reconstruction with allograft or autograft with or without augmentation. We report such a case of neglected patellar tendon injury treated with peroneus longus autograft.
    UNASSIGNED: A 37-year-old male patient presented with limping and inability to do a full extension of the knee. There is a history of a lacerated wound over the knee following a bike accident. Reconstruction was done with peroneus longus auto graft by making a trans-osseous tunnel through the patella and tibial tuberosity in the Figure eight 8 manner and fixed by a suture anchor. Postoperatively, the patient was doing well at a 1-year follow-up.
    UNASSIGNED: Good clinical outcomes can be achieved in neglected patellar tendon rupture by auto graft without augmentation.

    We present a case report of a 43-year-old male patient with simultaneous ruptured of anterior cruciate ligament (ACL), patellar tendon (PT) and lateral meniscus tear, occurred during a tennis match. This combinate tear is not frequent, and literature reports only few cases. Clinical diagnosis can be difficult and the support of MRI and X-Ray is needed. Surgery was performed within seven days from injury in a one-step fashion. Clinical and radiological outcome treatment was successful.

Diagnosis of simultaneous PTR is crucial as it changes clinical management. It is, therefore, important to maintain a high index of suspicion for the combination of TTF and PT injury.
