    This review article attempts to analyze the various research studies conducted in developing the models to evaluate the anatomy of the middle ear, its biomechanics, and the applications of these models in normal and diseased states. Various studies conducted over the past 50-60 years have been critically analyzed. We also discuss the various advantages and disadvantages of different methods of measurement of middle ear parameters. Beginning from anatomical modelling to histopathological sections and the latest three-dimensional (3D) reconstruction with finite element modelling, various methods of middle ear measurements have been critically analyzed. At the end of this review, we have concluded that the best and most effective method of middle ear modelling is the 3D reconstruction using high-resolution computed tomography and finite element modelling.

    The isolated malleus handle and neck fractures are rare pathology which demands detailed diagnosis and individual treatment plan in every case. Sometimes it could heal without surgery, but in most part of cases surgical treatment is indicated. The short literature review and 2 clinical cases of this pathology are presented in this article. We describe our own variants of surgery repair with autologous cartilage with good results.

    Malleostapedotomy (MS) is an evolving otology procedure that involves attaching prosthesis to the malleus on one end and the stapes footplate on the other end. It has multiple indications and requires skills and experience. We present 2 cases operated in our center (King Abdullah Ear Specialist Center, Riyadh, Saudi Arabia) in which intraoperative incus injury occurred, and MS was used to reconstruct the ossicular chain with good postoperative outcomes and patient satisfaction. As the procedure was surgically difficult and time consuming, many otologists prefer not to do it. Nevertheless, we believe that all surgeons dealing with stapes surgery should be familiar and gain confidence in performing MS considering that intraoperative incus dislocation is a complication that can happen even with great caution.

    UNASSIGNED: Report a series of cases in which patients have concomitant superior semicircular canal dehiscence (SSCD) and a dehiscent tegmen tympani with Dural contact to the malleus head (DCMH).
    UNASSIGNED: An analysis of radiologic and audiologic data in 4 patients who presented with SSCD and DCMH at a tertiary care institution. A pertinent literature review was performed.
    UNASSIGNED: Four patients (5 ears) had SSCD and DCMH. In 3 patients with unilateral DCMH, the mean maximum air-bone gap was 15 dB in the ear with DCMH compared to 50 dB in the ear without DCMH. Of the 5 ears with DCMH, the mean air conduction threshold at 250 Hz was 17 dB compared to 42 dB in the 3 ears without DCMH.
    UNASSIGNED: We report the findings of DCMH in a series of 4 patients with bilateral SSCD. This limited series suggests that ears with SSCD and DCMH have less of an air-bone gap than would be expected, as 1 would expect an additive effect of DCMH and SSCD on the air-bone gap.

    Malleus fracture is a rare condition. Usually, the handle of the malleus is involved, and we do not find reports in the literature of this condition in the bilateral presentation. It is present as sudden conductive hearing loss commonly after digital manipulation of the external auditory canal. The diagnosis is based principally on clinical examination by otomicroscopy and audiometry. Cone-beam computed tomography emerging as a powerful tool in the field of otolaryngology, especially for explorations of paranasal sinuses and temporal bone, due to imaging with a high resolution and few artifacts with lower dose radiation in comparison with multislice computed tomography.

    OBJECTIVE: To compare post-operative audiometric outcomes for the two prevailing surgical approaches for isolated malleus and/or incus fixation: ossicular mobilisation with preservation of the ossicular chain, and disruption and reconstruction of the ossicular chain.
    METHODS: A search was conducted, in December 2016, of PubMed, Scopus, and Cumulative Index to Nursing and Allied Health Literature articles written in English. Papers presenting original data regarding post-operative audiometric outcomes in patients who underwent surgical treatment for malleus and/or incus fixation with a mobile and intact stapes were included. A risk of bias assessment was performed on the 14 selected papers and a tier system was developed. Meta-analysis was accomplished by comparing pooled rates of surgical success by chi-square test and calculating odds ratios by logistical regression. Analysis was performed using Revman5 and R software.Results and conclusionAnalysis of the literature revealed no differences in audiometric outcomes between ossicular chain mobilisation and ossicular chain reconstruction in patients with isolated malleus and/or incus fixation. A large, prospective study comparing both short- and long-term hearing results for ossicular chain mobilisation and ossicular chain reconstruction in this population may identify whether a difference in outcomes exists between the two approaches.

    Objective To systematically review the anatomy of the ossicular chain. Data Sources Google Scholar, PubMed, and otologic textbooks. Review Methods A systematic literature search was performed on January 26, 2015. Search terms used to discover articles consisted of combinations of 2 keywords. One keyword from both groups was used: [ ossicular, ossicle, malleus, incus, stapes] and [ morphology, morphometric, anatomy, variation, physiology], yielding more than 50,000 hits. Articles were then screened by title and abstract if they did not contain information relevant to human ossicular chain anatomy. In addition to this search, references of selected articles were studied as well as suggested relevant articles from publication databases. Standard otologic textbooks were screened using the search criteria. Results Thirty-three sources were selected for use in this review. From these studies, data on the composition, physiology, morphology, and morphometrics were acquired. In addition, any correlations or lack of correlations between features of the ossicular chain and other features of the ossicular chain or patient were noted, with bilateral symmetry between ossicles being the only important correlation reported. Conclusion There was significant variation in all dimensions of each ossicle between individuals, given that degree of variation, custom fitting, or custom manufacturing of prostheses for each patient could optimize prosthesis fit. From published data, an accurate 3-dimensional model of the malleus, incus, and stapes can be created, which can then be further modified for each patient\'s individual anatomy.

    The ossicular chain has been known for 500 years and yet there are a small number of morphometrical studies. We reviewed the whole literature that is available online regarding the human ossicular chain from an anatomist perspective and correlated the data from all the papers that showed any relevance. Inclusion and exclusion criteria were developed a priori. A thorough description of all ossicular differences has been made and we present their variations in dimensions trying to associate measurements obtained with race. This research included papers spreading on a horizon of over 50 years of worldwide experience. Statistical analysis revealed that there is a great difference in measurements and the results cannot be sufficiently associated. The explanation of this variation in the measurements obtained might be due to errors in the procedure. We conclude that ossicular chain reveals a great variety, and propose that a measurement protocol for auditory ossicles must be widely performed.

    Behavioral and anatomical studies relating to possible seismic sensitivity in the desert golden mole (Eremitalpa granti) are reviewed. Field studies in the Namib desert have shown that isolated hummocks of dune grass generate low-frequency vibrations, distinct from the background noise at a distance of many meters. The golden mole apparently uses these cues to orient itself toward the hummocks and the prey species within. An analysis of middle ear morphology suggests that the massive malleus of the golden mole is adapted toward a form of inertial bone conduction, suitable for the detection of seismic cues obtained in this manner. The significance of seismic sensitivity in this golden mole is briefly discussed.
