nurse residency programs

  • 文章类型: Journal Article
    方法:此范围审查是由JBI(以前称为JoannaBriggsInstitute)的范围审查方法提供的。
BACKGROUND: The American Nurses Credentialing Center\'s (ANCC\'s) Practice Transition Accreditation Program (PTAP) establishes standards for nurse residency programs to elevate and optimize the skills, knowledge, and attitudes of new nurses participating in nurse residency programs. Evidence-based practice (EBP) is foundational to providing safe nursing care. One of the National Academy of Medicine\'s (NAM\'s) 2020 goals stated that 90% of clinical decisions would be supported by the best available evidence to attain the best patient outcomes. Nurse residency programs can benefit from evidence-based strategies to develop EBP competencies in new nurses.
OBJECTIVE: The purpose of this scoping review was to synthesize the literature around strategies for incorporating EBP into nurse residency programs across the United States.
METHODS: This scoping review was informed by the JBI (formerly known as the Joanna Briggs Institute) methodology for scoping reviews. Searches were conducted by a health science librarian in PubMed and CINAHL with Full Text. Keywords and their synonyms, Medical Subject Headings (MeSH; PubMed), and Subject Headings (CINAHL with Full Text) were used. Covidence, a literature review management program, was used to organize the literature and manage the review. Title, abstract, and full-text reviews were completed within Covidence using three teams of two independent reviewers.
RESULTS: Four hundred and thirty-eight citations were imported into Covidence. Ten articles were retained for the final review. Three strategies for incorporating EBP into nurse residency programs emerged from the literature: (1) exposure of nurse residents to existing organizational resources, (2) completion of online EBP modules, and (3) completion of an EBP project.
UNASSIGNED: The incorporation of EBP competencies in nurse residency programs aligns with NAM\'s and ANCC\'s goals, yet a paucity of evidence exists to guide curriculum development in nurse residency programs. This scoping review corroborates the need for further research to inform best practices for implementing EBP into nurse residency programs.

To determine the stress experience and coping styles of new nurses during Nurse Residency Programs (NRPs) by identifying, appraising, and synthesizing data from the qualitative studies.
A systematic review and meta-synthesis of qualitative studies.
Eleven databases were systematically searched for relevant publications in March 2022. All qualitative and mixed-method studies in English and Chinese that explored the stress and coping experience during NRPs of new graduate nurses were included. The qualitative meta-synthesis was performed following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) recommendations. Two independent reviewers selected the studies and assessed the quality of each study. Meta-synthesis was performed to integrate the results.
A total of 13 studies revealed 13 sub-themes and three descriptive themes: multi-dimensional stressors, somatic and emotional responses, coping resources and coping methods.
New nurses faced a lot of physical and emotional stress during NRPs, which had a negative impact on their physical and mental health. NRPs are a critical period for the career growth of new nurses. Effective management strategies must be implemented to improve nurse capacity, meet their needs, improve self-efficacy, and build organizational support, as this can improve the quality of clinical nursing and keep the enthusiasm and stability of the nursing team.

With the prevalent problem of nurse retention and the lack of confidence new nurses experience, it is critical for nurse residency programs to have supportive factors that enhance residency experiences. By incorporating mentorship, preceptors, and debriefing components into these programs, skills in communication, organization, clinical reasoning, stress management, and self-confidence increase. This quantitative study was a descriptive analysis of secondary data using a cross-sectional design. Two instruments were used to gather data, the Demographic Information Survey and the Evaluation of the RN Residency Survey. The sample was comprised of 1078 graduate nurses completing nurse residency programs throughout the United States. The findings indicated that higher residency ratings were significantly related to higher preceptor and debriefing experiences. Results also showed no significant interaction effects of reduced stress due to mentoring on the relationship between preceptorship experiences or debriefing experiences and ratings of nurse residency. Consequently, it is important that new nurses are given the opportunity to take part in programs offering mentorship, preceptors, and debriefing elements, as nursing residents have found them to be highly beneficial. Furthermore, debriefing sessions strengthened their skills and built confidence during their first year of employment.

Due to the continued shortage of public health nurses, some local public health agencies have begun hiring new graduate baccalaureate nurses into the public health nurse role. These new graduates require an increased level of support for transition to practice.
The goal of this project was creation of a transition to practice program designed specifically to meet the needs of new graduates hired in public health settings.
The core competencies of public health nursing were used as a framework to develop this residency program. A group of public health staff, supervisors, and faculty met monthly for three years to develop this program. Key features include general orientation, preceptors, looping experiences, case studies and peer support.
The program is available as a web resource beginning spring 2018. In order to evaluate this program, data on job satisfaction, employee retention and level of competence in the core competencies of public health will occur using instruments administered prior to beginning and immediately following completion of the new graduate residency.
The components of the program mirror best practices for new graduate residencies and are based on core competencies for public health nursing. This residency program is an important step for enhancing the professional development of new baccalaureate graduates in public health settings, and preparing the next generation of public health nurses.
Through the increased support and intentional education of the residency program, public health agencies will be able to attract and retain new graduates who develop the essential knowledge and skills to provide safe and effective care in the public health setting.

This paper presents the results of a national survey of pediatric nurse residency programs (NRP).
METHODS: The Pediatric Nursing Certification Board (PNCB) database was used to identify 316 hospitals with pediatric units, including children\'s hospitals and community hospitals with pediatric services. The Residency Task Force of the Institute of Pediatric Nursing (IPN) developed the survey, Exploring Pediatric Nurse Residency Programs. Survey items addressed structure, content, outcomes, benefits and challenges of NRPs, including a comparison with orientation programs and use of preceptors.
RESULTS: Of the 316 hospitals contacted, 65 provided usable information and 45 reported having an NRP. Most (94%) of the hospitals have an orientation program, and 70% had an NRP. The NRPs were typically internally developed (60%) and a year in length (46.5%). Most common content (>80%) included critical thinking, stress management, small group support, professional role transition, pediatric resuscitation, and evidence based practice. Evaluation of the NRPs included measures of satisfaction, turnover rates, and standardized measures, primarily the Casey-Fink Graduate Nurse Experience Survey (48.7%). Challenges include obtaining financial support from the organization, developing content relevant across units, providing time away from clinical units, and maintaining preceptors. Benefits noted included development of professional role confidence and peer support networks, increased safe nursing practices, and a decrease in nursing turnover.
CONCLUSIONS: In the ongoing development of NRPs in children\'s hospitals, issues such as appropriate content, optimal length, standardization across settings, impact on nurse retention, safe practice and patient outcomes all need to be addressed.

The 2010 Institute of Medicine (IOM) report, The Future of Nursing: Leading Change, Advancing Health, challenges the nursing profession to take a key role in redesigning the health care system. Intended to shape the future of nursing in the United States, the IOM report has implications for nursing worldwide. While individual states and nursing organizations are developing initiatives to implement the IOM recommendations in the United States, there must be a concomitant effort to examine the ripple effect on global health and the nursing community. This article addresses four IOM recommendations that are directly relevant to internationally educated nurses who practice across borders: nurse residency programs, lifelong learning, leading change to advance health, and interprofessional health care workforce data. The article discusses the IOM recommendations through a global perspective and offers policy implications for legislators, health care organizations and nurse educators, regulators and administrators.

All nurses should practice to the full extent of their education and training. This key message from the Institute of Medicine report, The Future of Nursing: Leading Change, Advancing Health, applies to all RNs. Barriers need to come down that prevent nurses from reaching their full potential at a time when the US health care system needs to be operating at full capacity. With the focus of US health care shifting toward promoting wellness, preventing disease, and managing chronic conditions better, the role that nurses play in coordinating care will increase. As key members of health care provider teams, nurses\' contributions to the delivery of safe, high-quality, cost-effective, and patient-centered care must be recognized and valued. For nurses to realize their full potential in new models of health care delivery, a transformation of nursing practice will need to occur. This overview demonstrates how professional nurses should prepare and contribute to critical changes needed in health care reform.
