    Poxviridae have been successful pathogens throughout recorded history, infecting humans among a variety of other hosts. Although eradication of the notorious smallpox has been a globally successful healthcare phenomenon, the recent emergence of Monkeypox virus, also belonging to the Orthopoxvirus genus and causing human disease, albeit milder than smallpox, is a cause of significant public health concern. The ongoing outbreak of monkeypox, demonstrating human-human transmission, in previously nonendemic countries, calls for critical need into further research in the areas of viral biology, ecology, and epidemiology to better understand, prevent and treat human infections. In the wake of these recent events, it becomes important to revisit poxviral infections, their pathogenesis and ability to cause infection across multiple nonhuman hosts and leap to a human host. The poxviruses that cause human diseases include Monkeypox virus, Molluscum contagiosum virus, and Orf virus. In this review, we summarize the current understanding of various poxviruses causing human diseases, provide insights into their replication and pathogenicity, disease progression and symptoms, preventive and treatment options, and their importance in shaping modern medicine through application in gene therapy, oncolytic viral therapies for human cancers, or as poxvirus vectors for vaccines.

    Molluscum contagiosum and scabies are contagious conditions that can be acquired by incidental casual contact of a disease-free individual with an infected person. However, both the viral infection and the mite infestation can be transmitted sexually from the infected person to the unsuspecting recipient partner. A 20-year-old man without any infectious diseases acquired not only molluscum contagiosum but also scabies after a single sexual encounter with a female partner; hence, he developed two sexually transmitted diseases after one sexual episode: two for one. He presented for medical attention to evaluate the new white umbilicated papules on his penile shaft and red nodules on the corona and glans of his penis. A complete skin examination also revealed additional papules on his abdomen and suprapubic region and a burrow on his left index finger adjacent to the finger web. Diagnosis of the molluscum contagiosum infection was confirmed by observing molluscum bodies on the molluscum preparation taken from an abdominal papule and the scabies infestation was confirmed by noting not only mite eggs but also mite scybala (feces) on the scabies (mineral oil) preparation from the burrow. The molluscum contagiosum infection was treated with curettage of the lesions. The scabies infestation was treated with two treatments (each one week apart) of permethrin five percent cream overnight topical application from his neck to his toes. All his lesions resolved and did not recur. In conclusion, the development of a new sexually transmitted disease in an individual should prompt the clinician to evaluate the patient for additional sexually transmitted diseases since the patient may have acquired more than one infection from their partner during the sexual encounter.

    BACKGROUND: VP-102 is drug-device combination product containing cantharidin (0.7% w/v) and has undergone Phase III clinical trials for the treatment of molluscum contagiosum (molluscum). Efficacy and safety may differ by body region due to variable skin anatomy.
    OBJECTIVE: We investigated the pooled safety and efficacy of VP-102 by affected body region.
    METHODS: Individuals at least two years of age with molluscum were randomized to topical VP-102 or vehicle once every 21 days until clear (maximum of four applications). Participants were assigned to body region groups where lesions were present at baseline. Body region lesion counts were recorded at each visit. Efficacy was measured by the percentage of participants with complete clearance of lesions by region. Pre-specified adverse events (AEs) were analyzed for those treated in the region on that visit.
    RESULTS: Participants had a mean of two regions affected at baseline. Complete clearance was significantly higher in the VP-102-treated group than with vehicle application in all regions at the last visit (P<0.01 for each region). The incidence of pre-specified AEs was consistent across regions. However, these analyses were post hoc, and individual lesions were not tracked for efficacy.
    CONCLUSIONS: VP-102 treatment shows consistent safety and efficacy across molluscum body regions.

    Many over-the-counter (OTC) products are marketed to treat molluscum contagiosum, a common childhood viral infection with an estimated point prevalence of 5%-11%. As optimal treatment has not been defined, a comprehensive internet search was performed to identify OTC products marketed for treating molluscum and to collect information on packaging, ingredients, application instructions, and dosing. Nine OTC products were identified and varied widely in cost, packaging size, and ingredients; six of the nine products contained Thuja occidentalis. Overall, the majority of OTC package labels do not include sufficient information about their plant-based ingredients or appropriate dosing.

    Infectious cutaneous diseases are very common, especially in certain geographic and tropical regions. Sometimes they may simulate other dermatoses, ordering verification of diagnosis with particular investigations. Dermoscopy is among one of the most important tools readily available in the outpatient setting for the dermatologist to confirm the diagnosis. In this up-to date review, literature concerning the various dermoscopic features of parasitic, viral, dermatophytic and bacterial cutaneous infections is composed. In addition artefacts as well as practical issues in dermoscopy usage are discussed; with the aim to empower dermatologists to promptly and non-invasively diagnose and manage cutaneous infections and infestations.

    Molluscum contagiosum (MC) is a common cutaneous viral infection with no standard treatment. The virus responsible for MC is thought to be cleared by cell mediated immunity (CMI). Intralesional immunotherapy that stimulates CMI has been shown to be an effective treatment for other cutaneous viruses. In this review, we evaluate the efficacy and safety of intralesional immunotherapy in the treatment of MC. Articles met inclusion criteria if they examined the effects of intralesional immunotherapy as a treatment for MC, with a clear outcome and reproducible methodology. 228 studies were screened and 10 studies met criteria for inclusion. Intralesional immunotherapies investigated included candida, combined measles, mumps, rubella vaccine, tuberculin purified protein derivative, vitamin D3, interferon α, and Streptococcal substrain OK-432. Studies demonstrated clearance of MC lesions following intralesional immunotherapy, with complete response rates between 36% and 100%. No serious adverse effects were noted. Intralesional immunotherapy is a safe and effective treatment option for MC in pediatric and adult patients.

    Molluscum contagiosum (MC) is a self-limiting viral infectious cutaneous disease usually characterized by pinkish skin color and round umbilicated papules varying in size and shape. MC can affect any region, but the lower abdomen, thighs, genitals, and perianal area are the sites most commonly involved. Atypical localizations of the lesions may be difficult to diagnose. We report a case of subungual MC with dermoscopic features in a young male.
