    Isolated volar distal radioulnar joint (DRUJ) dislocation is a rare condition with only a few cases reported in the literature. Its diagnosis is often overlooked in the acute phase, and there is no consensus in its management. We present the case of a 20-year-old male patient with an isolated volar radioulnar dislocation, together with a review of the literature. The aim is to present and summarize the acute management of this condition and propose a therapeutic algorithm.

    OBJECTIVE: Lymphomas associated with breast implants are mostly of the T-cell type which typically present as a late-onset seroma or palpable mass adjacent to the implant. Primary lymphomas of the breast, in the absence of breast implants, are mostly of the B-cell type. However, we present a case of an Epstein-Barr virus-positive diffuse large B-cell lymphoma in a patient with polyurethane textured implants.
    METHODS: A 75-year-old woman presented with a sudden onset of swelling in her right breast. Her medical history revealed a unilateral mastectomy at the age of 48 for an invasive ductal adenocarcinoma in her left breast. Reconstruction was performed using bilateral McGhan style 150 implants. Nine years later, she was diagnosed with Baker IV capsular contracture and bilateral rupture, as seen on magnetic resonance imaging. A complete capsulectomy with implant change (Polytech, Replicon® SL HP) and mastopexy on the right was performed. Given her history and the sudden onset of swelling, the situation was concerning. Ultrasound showed a large mass in contact with the implant and an effusion around it. After mastectomy with explantation and capsulectomy, she was diagnosed with an Epstein-Barr virus-associated diffuse large B-cell lymphoma (DLBCL) of the capsule in association with textured breast implants.
    CONCLUSIONS: We report the first case of polyurethane textured implant association with the rare entity of EBV+ DLBCL. Our aim is to renew awareness of the clinical significance of late periprosthetic seroma and highlight the need for documenting all cases to advance our understanding of breast implant-associated lymphoma.

    Bacteria resistance to antibiotics has emerged as a major health problem. Developing new antibacterial systems is then of major interest. In this sense, we present biocapsules presenting inherent antibacterial capacity. The self-assembly of charged biopolymer, namely diethylaminoethyl-dextran hydrochloride (dex+) and dextran sulfate (dex-), were done on calcium carbonate microparticles, used as a template. Zeta potential measurements have shown the successful alternate adsorption of these biopolymers and related charge reversal upon the multilayer film construction onto the particles surface. The shape of the capsules was characterized by scanning electron microscopy (SEM). These particles were tested against bacteria resistant to antibiotics, namely kanamycin-resistant Escherichia coli. An inhibitory effect of the particles was observed during bacterial growth in liquid medium, i.e. in the range of 10 % for (dex+/dex-)n coated CaCO3 materials and of 50% for (dex+/dex-)n capsules. These findings evidence the high potential of capsules to act as antimicrobial agents in future and in treatments against infections.

    Renal leiomyoma is a rare benign mesenchymal tumor of the kidney that predominantly originates from the renal capsule or pelvis. However, because of its nonspecific clinical and imaging features, renal leiomyoma remains poorly characterized and may even lead to radical or partial nephrectomy on the basis of preoperative suspicion of renal carcinoma. We herein present a case involving a 12-year-old boy with acute abdominal pain who was diagnosed with renal leiomyoma based on both clinical imaging and histopathological examination. One year after radical nephrectomy, the patient recovered to good condition. This case demonstrates that the comprehensive application of imaging and histology are essential for early clinical diagnosis and effective treatment of renal leiomyoma.

    UNASSIGNED: Noninvasive encapsulated anaplastic thyroid carcinomas (NE-ATCs) have been described in few case reports, and consistently associated with favorable outcome compared to the classical ATCs.
    UNASSIGNED: Our aim is to remark a rare histological finding in ATCs, encapsulation, which has been associated with a favorable outcome.
    UNASSIGNED: We have documented a rare case of an NE-ATC with its clinical, pathological, and molecular features. We also provided a thorough discussion of all the encapsulated ATCs reported in the literature.
    UNASSIGNED: A 50-year-old woman with an unremarkable medical history, who presented with a thyroid nodule, and diagnosed as \"follicular lesion of undetermined significance\" by fine needle aspiration biopsy. The patient was lost to follow-up for six years and revisited upon her neck disturbances and underwent total thyroidectomy.
    UNASSIGNED: Sections of the right lobe revealed a grossly encapsulated nodular lesion, measuring 75x55x55 mm. Histologically, the tumor consisted of both carcinomatous and sarcomatous components supported by immunohistochemical stains. Necrosis and atypical mitotic figures were evident. Capsular and/or vascular invasion was not identified. There were no BRAF codon 600, KRAS, NRAS mutations and RET/PTC rearrangement. During three-month follow-up, the patient was free of disease without adjuvant therapy.
    UNASSIGNED: Encapsulated ATCs tend to follow a favorable clinical course and may deserve conservative treatment approaches.

    BACKGROUND: The Elipse™ intragastric balloon (IGB) for weight loss is a swallowable capsule that is filled with 550mL of fluid and resides in the stomach for four months before being excreted from the gastrointestinal tract. Although initial data showed that use of this device is safe and free from serious complications, we report for the first time the successful management of an Elipse™ IGB-related adverse event.
    METHODS: A 41-year-old woman presented to our emergency department following two days of abdominal pain, vomiting, and constipation. Her medical history included four caesarean sections and insertion of the Elipse™ IGB 16 weeks prior to presentation. The patient was vitally stable at presentation and abdominal examination revealed a mildly distended abdomen. Plain X-ray revealed a small bowel obstruction (SBO), and a double contrast computed tomography scan showed a dilated small bowel with mild free fluid proximal to a transition zone at the distal jejunum. Laparoscopic enterotomy was performed just proximal to the obstruction site, and the balloon was visualized and extracted after it had been incised and emptied. The enterotomy incision was closed with an intracorporeal continuous absorbable suture. The patient\'s recovery was uneventful and she was discharged on postoperative day 4.
    CONCLUSIONS: We discuss the possible etiologies of SBO following Elipse™ IGB insertion, and present a brief literature review regarding surgical and nonsurgical management options for such cases.
    CONCLUSIONS: Although initial data showed the Elipse™ IGB to be safe, complications can occur and be managed successfully.

    An acute upper gastrointestinal bleeding event is an emergency situation which requires immediate endoscopic assessment and treatment. A new telemetric real-time intracorporeal bleeding sensor can help in the timely diagnosis of an acute upper gastrointestinal bleeding event: The sensor capsule, HemoPill, is swallowed by the patient if gastrointestinal bleeding is suspected. Information about the bleeding status is displayed by telemetric communication of the capsule with an extracorporeal receiver. This is the first evaluation of the HemoPill to detect blood in the upper human gastrointestinal tract.
    A voluntary test person ate a defined meal with or without the adjunct of his own blood for a total of eight times and afterward swallowed the sensor capsule. The collected spectrometric receiver data were analyzed to assess whether the sensor system was capable of detecting blood and to evaluate the effect of stomach content as an artifact.
    With its optical sensor, the HemoPill was able to reliably indicate the ingested blood in the stomach. The data transmission from the swallowed sensor capsule to the extracorporeal receiver was achieved consistently. The evaluation of diverse concentrations of ingested blood and the respective sensor signals led to an exponential relationship of these variables. This relationship allows to define thresholds for categories indicating the likelihood of blood presence in the gastrointestinal tract.
    The HemoPill is a valuable tool to detect an acute upper gastrointestinal bleeding event without the need of endoscopy.

    Invasive meningococcal diseases are rarely caused by capsule null locus (cnl) strains, which are unencapsulated owing to an absence of capsule synthesis genes. Only a few cases of cnl meningococcemia in severely immunocompromised patients have been reported. Here, we describe and discuss the first case of invasive disease in an immunocompetent patient due to a cnl strain of Neisseria meningitidis in China. We characterize this strain and discuss the special ST-198 complex of cnl.
